Human review is not failure
Teams still talk about review as if it proves the system is not mature enough. That framing is backward. In many workflows, review is the design that makes the system trustworthy enough to use.
The question is not whether a human is present. The question is where the human should step in: before action, after draft generation, at exception points, or only when confidence drops.
Better pattern
A system that is explicit about draft mode, approvals, escalation, and what happens when inputs are messy.
